Ms. Pallister’s background as a Home Health Case Manager makes her uniquely qualified to prepare Life Care Plans. Having visited hundreds of clients in their homes, Ms. Pallister has the assessment skills of a seasoned professional. She knows what to assess in the physical surroundings of the home, and what to ask to clearly gain an understanding of how her clients and their families are coping after leaving the security of an in-patient setting. Having worked across the continuum of care (in acute care, long term care and outpatient settings) for more than 15 years, and always within a multidisciplinary team, Ms. Pallister has the experience essential to seeing the broader picture. Her solid nursing background gives her the expertise needed to create a plan that realistically forecasts the future needs of her clients.
